If you are porting from Google Voice, please provide a screenshot of the homepage of your Google Voice account that includes your number. Also, make sure to unlock the number per Google Voice's port out policy

We will provide you a Letter Of Authorization (LOA) for you to sign and return to us so we may proceed with the porting process
